1. Submit the Authorization Form

2. Pay the Processing Fee
After receiving the form, we'll contact you if there is any fee to be paid.
3. Receive Records via Mail, Fax or Email
As you fill out the form, select the delivery method of your choice and we'll gladly send it over.

Yes, you may submit a paper medical release form to us, but please know that it may take longer to fulfill your request. We strongly encourage all patients to submit an electronic form to speed up the process.
